PRIVATE DUTY  MANAGER

RN OR LPN POSITION

Full-Time | Daytona Beach

We are a home health care provider looking for an Licensed Practical Nurse/Registered Nurse to join a fast-growing Joint Commission Accredited Agency here in Volusia and Flagler counties as a Private Duty Manager! This position is perfect for a LPN/RN looking for a change of pace. Our Private Duty Manager will be providing oversight on day to day operations for a segment of patients and employees. Duties will be performed primarily in our office located in Daytona Beach, but the applicant should also expect to see home care patients as needed.

Responsibilities:
– Regulation compliance
– Monitoring and oversight of CNA’s + HHA’s
– Development and implementation of schedules
– Conduction of home and community-based assessments
– Development of patient profiles
– On-site patient care
– On-call required

Qualifications:
– RN or LPN
– Previous experience with Joint Commission
– Familiarity with medical software
– Ability to provide on-site patient care
– Self-starter possessing the ability to work independently

We are looking for a candidate that will do whatever it takes to find solutions and methods of implementation to make their department run smoothly. Applicant should be able to set goals, complete tasks, and stay focused without rigid guidelines. HOME HEALTH AID

Part-Time | Daytona Beach

Nurses Aides perform the following activities in the maintenance of the health care for in the home patients.

➢ Assist with:
• Baths, Oral Hygiene, Eating, Active + Passive exercises, Rehabilitation,

Grooming needs, Ambulation + Transfer Activities.

  • ➢  Keeping daily record of pertinent observations and activities + report any incidents or changes in patient’s behavior to Agency supervisor.
  • ➢  Keeping patient’s belongings + clothes clean, including light housekeeping.
  • ➢  Prepare nutritious meals and encourage nutritional support.
  • ➢  Provide companionship and maintain confidentiality of patient’s illness.
  • ➢  To provide escort service such as MD appointments, trips + outings.
  • ➢  Supervision of self-administered medication in the home limited to the following:

Obtaining the medication container from the storage area. Preparing necessary liquids to assist the patient with self-administration. Reminding the patient that it is time to take medication. Observing the patient self-administer the medication

  1. Assist with Personal Care:Bathing, Grooming, Oral Care, Dressing.
  2. Assist with Mobility Needs:Safe Ambulation / Transfers, prescribed exercises, use all adaptive equipment.
  3. Nutrition:Plan, prepare and serve meals, offer snacks, encourage hydration, clean up.
  4. Medication Reminders
  5. Light Housekeeping:Counters, sweep, vacuum, bed care, client’s laundry. Maintenance of client’s personal care areas, and provide safe and sanitary environment.
  1. Shopping:
    For the client, pick up medications.
  2. Transport: As per Contract.
  3. Record daily care per individualized care plan.
  4. Provide companionship and confidentiality.

CERTIFIED NURSING ASSISTANT
